How to Calculate IV Flow Rate in mL per Hour
An infusion-rate question gives a volume and a duration, then asks how much of that volume corresponds to one hour. The arithmetic is division, but the time unit must be correct before the calculation begins.

Formula
rate in mL/hour = volume in mL ÷ time in hours
Start with the requested unit
If the question asks for mL/hour, the time used in the denominator must be expressed in hours. A duration of 30 minutes is 0.5 hours; 45 minutes is 0.75 hours; 15 minutes is 0.25 hours.
Dividing by a duration shorter than one hour can produce a rate larger than the original volume. That is mathematically possible because the result describes an hourly equivalent, not the amount already delivered.
Worked example: whole hours
For a fictional 1,000 mL volume over 8 hours, divide 1,000 by 8. The result is 125 mL/hour. A useful sense-check is that eight groups of 125 mL return to 1,000 mL.
Worked example: minutes
For a fictional 100 mL volume over 30 minutes, first convert 30 minutes to 0.5 hours. Then calculate 100 ÷ 0.5 = 200 mL/hour. Multiplying 200 mL/hour by 0.5 hours returns to the original 100 mL.
Common errors
- Dividing by 30 while labelling the result mL/hour.
- Multiplying volume by time instead of dividing.
- Converting 45 minutes to 0.45 hours rather than 0.75 hours.
- Rounding too early and carrying an avoidable error into the final answer.
- Treating the arithmetic result as a clinical instruction.
